We will prorate your final payment to only cover the lessons within the 30-day notice which is marked from the date of your email. Once you send the completed form, you will receive an auto reply confirming we have received it. If you do not receive this confirmation email, please email us directly at email@example.com. A representative will email you with final billing and scheduling details within 48-72 hours.

Please note that students who re-enroll within 3-months of discontinuing do not have to pay any registration fees!
